Premature ejaculation. Common, defined, and treatable.
Short answer: premature ejaculation (PE) is ejaculating sooner than you or your partner want, with little sense of control, and feeling distressed about it. Clinically, lifelong PE means ejaculating within about one minute of penetration almost every time. It affects roughly 20 to 30% of men at some point, and most cases respond well to behavioral techniques, treatment of underlying causes, or medication.
What counts as PE
The International Society for Sexual Medicine defines PE with three parts: short time to ejaculation (about 1 minute for lifelong PE, or a clear drop to 3 minutes or less for acquired PE), inability to delay it on most occasions, and negative consequences such as distress or avoiding intimacy. Occasionally finishing fast is not PE.
Lifelong vs acquired
Lifelong PE has been present since your first sexual experiences and is thought to have a strong neurobiological component, including serotonin signaling. Acquired PE starts later and usually has a trigger: performance anxiety, relationship stress, erectile difficulties, thyroid problems, or prostatitis.
The ED connection
Many men with acquired PE also have early erectile difficulties. Fear of losing the erection pushes them to rush, which trains a faster response. Treating the erection issue often fixes the timing. If your PE started alongside softer erections, look at erectile function first.
What works
Behavioral methods (stop-start and squeeze techniques) help many men, especially combined with pelvic floor training. Topical anesthetic sprays or creams reduce sensitivity. Daily or on-demand SSRIs, such as dapoxetine where approved, are the most studied medication. In the US, these are used off-label and need a doctor's prescription.
When to see a doctor
If PE causes ongoing distress, appeared suddenly, or came with pain, urinary symptoms or erection changes, see a doctor or urologist. Tracking how often it happens and what precedes it (stress, alcohol, time since last ejaculation) makes that conversation far more useful.
