Exercise and erections. The dose that actually works.

Short answer: aerobic exercise is one of the best-supported natural treatments for erectile dysfunction. A 2018 review found that about 40 minutes of moderate to vigorous activity, 4 times a week, for 6 months improved erectile function. Pelvic floor training and strength work add to it. Chronic overtraining can do the opposite.

Why exercise works

Erections are a blood vessel event. Exercise improves endothelial function and nitric oxide availability, the same pathway that drugs like sildenafil act on. It also lowers blood pressure, improves insulin sensitivity and helps weight loss, all major ED risk factors.

The evidence-based dose

A systematic review by Gerbild et al. (Sexual Medicine, 2018) concluded that 160 minutes per week of moderate to vigorous aerobic exercise for 6 months reduced erectile problems, especially in men with obesity, hypertension, metabolic syndrome or heart disease. Brisk walking, cycling, running and swimming all count.

Strength training

Resistance training builds muscle, improves insulin sensitivity and can modestly support testosterone, especially when it helps you lose fat. Two to three full-body sessions a week is a good complement to cardio.

Pelvic floor training

In a UK randomized trial (Dorey et al., 2005), 40% of men with ED regained normal function after 3 months of pelvic floor exercises combined with lifestyle advice. These muscles help keep blood trapped in the penis.

When training hurts

Very high endurance volume without enough food or sleep is linked to lower testosterone and libido in male athletes. Long hours on a hard bike saddle can compress nerves and arteries; a well-fitted saddle and standing breaks help. If libido drops when training ramps up, that's a recovery signal.
