Short answer: the most specific signs of low testosterone are low sexual desire, fewer morning erections and erectile problems. Fatigue, low mood, weight gain and poor focus are common too, but they have many other causes. Diagnosis needs symptoms plus two early-morning blood tests showing low levels.
The three most specific signs
In the European Male Ageing Study (over 3,000 men), the symptoms most tightly linked to low testosterone were sexual: reduced libido, fewer morning erections and erectile dysfunction. When all three appear together alongside low blood levels, the diagnosis of late-onset hypogonadism becomes likely.
Common but less specific signs
Fatigue, low motivation, low mood, reduced muscle mass, more belly fat, poorer sleep and trouble concentrating. These are real effects of low testosterone, but they overlap heavily with poor sleep, stress, depression, thyroid problems and simply being overtrained. Don't self-diagnose from this list alone.
How it's diagnosed
Guidelines from the Endocrine Society and American Urological Association recommend measuring total testosterone in the morning (roughly 7 to 10 am), on two separate days, because levels swing through the day. A total below about 300 ng/dL combined with symptoms is the usual threshold. Doctors often also check LH, free testosterone and prolactin.
Common causes in younger men
Obesity is the biggest modifiable driver. Sleep apnea and chronic short sleep, heavy drinking, opioid use, past anabolic steroid use, type 2 diabetes and some medications also lower levels. Genuine testicular or pituitary problems exist but are less common.
What raises it naturally
Losing excess weight has the strongest evidence. Sleeping 7 to 9 hours, strength training, cutting alcohol and managing stress all help. Supplements marketed as 'test boosters' have weak evidence; zinc or vitamin D only help if you're actually deficient.
Should you consider TRT?
Testosterone therapy is for confirmed hypogonadism, not borderline numbers. It can reduce fertility, so men who want children need a different approach. It's a decision for a doctor after proper testing.
